Restore Hope Ministry School Supply Registration
In an effort to accommodate more families and schedules, Restore Hope Ministries will be registering for school supplies ONE WEEK ONLY but with extended hours and one Saturday.

Parent(s) or Guardian must have picture ID and social security card for themselves and social security cards and proof of birth date (birth certificate, shot records, school enrollment form, etc.) for each of their school aged children.
Assistance is limited to children in grades Pre-K – 12. Restore Hope Ministries does not assist with Head Start, private pre-school, home school, or private school. Pre-K students must be 4 years old before 9/1/2019. Kindergarten students must be 5 years old before 9/1/2019. DOB for eligibility: Pre-K -9/1/2014 – 8/31/2015; Kindergarten – 9/1/2013 – 8/31/2014; High School – earliest birth date 9/1/1998.
Restore Hope Ministries assists children enrolled in public school in Tulsa County, including: Berryhill, Bixby, Broken Arrow, Collinsville, Glenpool, Jenks, Owasso, Sand Springs, Skiatook (Tulsa County residents only), Sperry (Tulsa County residents only), Tulsa Public Schools, and Union Public Schools.
Parents must know which school their children will be attending in the fall in order to receive school supplies. Please check the list of schools or ask a staff member if you have any questions about the process. Contact Restore Hope Ministries at 918.582.5766.
